Getting Started
The Process begins with a no-cost phone inquiry initiated by a family member or one who is a close confidant of the client.
Once a decision is made to engage services, a date is conveniently scheduled for the in-person Care Assessment.*
Upon conclusion of the Assessment, a comprehensive Care Plan is compiled and timely sent to the client and/or the party representing the client.
It frequently becomes the responsibility of the Care Manager to implement the Care Plan.
The implementation process will vary according to client needs, treatments, home accommodations/support, or re-location.
*Definition: An evaluation in which multiple problems of older people are uncovered, described, and explained, if possible, and in which the resources and strengths of the person are catalogued, need for service assessed, and a coordinated care plan is developed.
